What is the Preventive Action for the Defects? Many professionals say “Provide Training to the Operators” as a standard response. This is vague and would not stop the defects from arising again.
How to create a Preventive Action?
Create Standards - Easy to understand standards to educate the employees on Do’s and Don’ts. Should be in local language / Images.
Create Visual Indicators - Visual Indicators alert the operators in case of any changes from the standards. E.g. Color coding of dies to avoid mixups
PokaYoke (Mistake Proofing) - Create PokaYoke systems that eliminates defects (would not allow errors to occur). e.g. only if the component is properly loaded, the machine will work, otherwise machine wont function.
Create Accountability Partners - form small teams who takes responsibility for each other’s adherence to the standards.
Rewards and Recognitions: Monitor the Progress and Reward the teams if they perform well
